Contracts for Services

In some cases we require completion of contracts for services to be performed. We include a list below for you to download in .pdf format. They have fields which can be filled out on-line and then print off the completed form. Alternatively you can download the form and complete and print off at a later date.
Once completed, please fax, e-mail or “snail mail” them back to us at the number or address included in the contract. If you have questions, please contact us!
Note that we must have these contracts completed fully completed and “in hand” before services will be rendered.

Management Contracts

These relate to mare breeding management and board, and stallion board. The forms should be completed, with areas referencing vaccinations etc. filled out and initialed where appropriate. If a horse has not been vaccinated or tested, do not initial that line.

Cooled Semen

It is crucial that when ordering semen – or canceling an existing order – our office is called by telephone and the person actually ordering the semen speaks to someone at Equine-Reproduction.com, LLC! Email, fax, text or left phone messages are not reliable ordering methods!

Please note that semen shipments must be ordered no later than 10:00 am Central Time on the day they are to be shipped or picked up. If an order is received after 10:00 am, if it can be accommodated, there will be a $150 “late order” surcharge. Semen pickup is after 2:00 pm, not before.

Canadian Shipments require additional (CFIA and USDA) documentation which we must have in hand prior to being able to ship. Please contact us for requirements specific to Canadian Shipments. We also outline current general protocols in our article about shipping semen to Canada. Note that for optimal success, we should be advised the day prior to the day of shipment for Canadian Shipments, which allows us to have all USDA documents prepared for an early collection and submission to USDA once our staff veterinarian arrives in the morning on the day of collection. While notification the day before shipment is not mandatory, if the submission to USDA is performed later in the day on the day shipment is desired, it is possible that we will not receive the endorsed Zoosanitary Certificate back in time to ship the semen that day. Note that on some Federal Holidays, even though FedEx may be operating, USDA will probably be closed.

Pre-shipment Contract (Cooled)
This contract must be sent ahead of a request for a cooled semen shipment from a stallion being managed by Equine-Reproduction.com, LLC. Note that in addition to the charges for the requested service, the authorized amount includes replacement value for the Equitainer above other charges. Your card will not be charged unless the Equitainer is not returned cleaned in a timely manner, in good condition and complete with all internal accessories.

Cooled Semen Evaluation
This contract should be sent ahead of time or along with a semen shipment which is being sent to us for evaluation, as is appropriate. Note that in addition to the charges for the requested service, the authorized amount includes replacement value for the Equitainer (if we provided one) above other charges. Your card will not be charged unless the Equitainer is not returned cleaned in a timely manner, in good condition and complete with all internal accessories.

Frozen Semen

Please note that frozen semen shipments can only be sent when vapor shippers are available. In the height of breeding season, people often have tendencies to wait until the last moment to order semen and then hold onto the shipper to “work” out of it (or for some other reason). This prevents others from getting semen when they need it. We strongly recommend that if you need frozen semen you order it well in advance of your anticipated insemination time, and also require that you only ship to a facility that is able to store the semen so that the shipper can be returned to us immediately that the semen has been transferred.

Pre-Shipment Contract (Frozen)
This contract must be sent ahead of a request for a frozen semen shipment being stored by Equine-Reproduction.com, LLC. Note that in addition to the charges for the requested service, the authorized amount includes replacement value for the vapor shipper above other charges. Your card will not be charged for replacement unless the Vapor Shipper is not returned in a timely manner, in good condition and complete with all internal accessories.

General Credit Card Authorization
